Qualifications & Requirements
- Working knowledge of admissions policies, procedures, and regulations within long-term care settings.
- Minimum of one year of experience in admissions, nursing, or a similar role in a long-term care or healthcare facility (preferred).
- Licensed Social Worker in Alabama (preferred).
- Education in Social Sciences or related field.
- Strong documentation, organizational, follow-up, and communication skills.
- Ability to market the facility to prospective residents, families, placement agencies, and the community.
- Excellent interpersonal skills with the ability to build strong relationships with residents, families, and staff.
- Demonstrated leadership ability and professionalism.
- Must be dependable, flexible, accountable, and demonstrate a positive attitude.
- Long-term care experience strongly preferred.
- Commitment to delivering excellent customer service and enhancing the highest practical quality of life for residents.
Essential Job Functions
- Interview residents and family members to obtain required admissions information, signatures, and social history details.
- Provide professional, welcoming tours of the facility while promoting Crowne Health Care's mission and services.
- Act as a liaison and advocate for the resident's emotional, social, and transitional needs throughout their stay.
- Assist in planning, organizing, implementing, and evaluating the Social Services program in accordance with federal, state, and local regulations.
- Maintain accurate and up-to-date listings of all residents, including level of care, room assignments, and admission/discharge status.
- Coordinate resident admissions, transfers, and discharges while keeping residents, families, and staff informed.
- Maintain required documentation such as admission/discharge logs, census reports, progress notes, and other departmental records.
- Ensure adherence to current Medicare and State regulations governing admissions, discharges, and resident rights.
- Build and maintain strong working relationships with residents, families, referral sources, and interdisciplinary team members.
- Advocate for and protect resident personal and property rights, ensuring all staff uphold these standards.
- Demonstrate exceptional customer service throughout the admissions process and in ongoing support of residents' social and emotional needs.
- Provide timely documentation, communication, and follow-up related to resident care, psychosocial needs, and service coordination.
