Charge Nurse (LVN/RN)

Crown Point Health Suites is a 108-bed rehabilitation and skilled nursing facility. We operate using a household model where residents live in one of four specialized houses: Sapphire, Emerald, Ruby, or Diamond. Each house contains its own kitchen, dining area, and living space. As a Charge Nurse, you will manage clinical outcomes within your assigned household, oversee the delivery of care, and guide the CNA team.

Pay, Schedule & Benefits

• LVN Starting Rate: $30/hr
• RN Starting Rate: $35/hr
• Schedule: 12-hour shifts; rotating weekends

Qualifications

• Valid Texas RN or LVN license in good standing
• Current BLS certification
• Minimum 6 months of nursing experience
• Graduate of an accredited school of nursing
• Experience in skilled nursing or post-acute rehabilitation preferred
• Successful completion of a post-offer health assessment

Skills

• Ability to supervise and lead a team of nursing assistants
• Strong clinical judgement and ability to remain calm under stress
• Positive communication skills for interacting with residents and families

Responsibilities

• Provide direct nursing care to residents according to physician orders and clinical standards.
• Supervise CNA staff in the daily delivery of resident care and hygiene.
• Administer medications and treatments accurately within the electronic medical record system.
• Conduct clinical assessments and monitor for changes in resident condition.
• Coordinate with the interdisciplinary team, including therapy and dietary, to meet care objectives.
• Communicate effectively with residents, families, and physicians regarding care plans.
• Ensure all local, state, and federal health regulations are met within your household.
