We are seeking a dynamic, compassionate, and highly organized Social Services Director to join our leadership team. This individual will play a key role in supporting residents and families, ensuring a smooth transition into our facility, and fostering a positive, person-centered experience throughout their stay.
What You’ll Do
- Oversee the Social Services department and ensure compliance with all state, federal, and facility regulations
- Provide support, counseling, and advocacy for residents and families
- Conduct assessments, develop individualized care plans, and participate in interdisciplinary team meetings
- Coordinate admissions, transfers, and discharges, ensuring clear communication and supportive transitions
- Facilitate care conferences and maintain accurate, timely documentation
- Build and maintain positive relationships with residents, families, staff, and community partners
- Address psychosocial needs and concerns, helping residents navigate challenges with dignity and respect
- Lead department staff, providing guidance, training, and performance oversight
- Promote a caring, therapeutic environment aligned with the mission and values of Swanton Valley
- Previous experience in long-term care or healthcare setting strongly preferred
- Strong leadership, organizational, and problem-solving skills
- Excellent communication and interpersonal abilities
