Position Description
- Reviews all incoming INR results and doses patients medication based off these results and the parameters set forth by the patient’s provider.
- Communicates directly with providers regarding critical out of range results.
- Reviews patient records for changes in condition documented by provider that may affect the efficacy of Coumadin. Adjust dosing accordingly in conjunction with the patients provider.
- Provides initial and ongoing education to patient and / or family member.
- Acts as resource for the Coumadin LPN, giving instruction as necessary.
- Communicates with outside agencies, patients and family members.
- Other duties as assigned.
- Education/Certification/Licensure
- Current Licensure as a Registered Nurse in the State of New York.
- Current Basic Life Support certification.
- Experience/Skills/Proficiencies:
- Bachelor’s Degree of Nursing from an accredited nursing school, preferred.
- Knowledge of medical terminology, anatomy, physiology and pathophysiology.
- Familiarity with health care systems, regulations, policies, and functions.
- Understanding of documentation standards.
- Knowledge of equipment, supplies, and materials needed for medical treatment.
- Works well in a team setting and demonstrates strong cooperation skills.
- Skill in reading and following written and oral medical orders.
- Skill in initiating appropriate emergency procedures.
- Skill in handling a number of tasks simultaneously.
- Experience in a physician office, preferred.
