Crouse’s Care Coordination Services team is hiring a Care Manager to review the services delivered to patients and compare to evidence-based guidelines in an effort to ensure medical necessity and appropriate level of care. This position will operate within the Crouse Health Neonatal Infant Care Unit (NICU).
Pay Range: $80,000.00 - $87,720.00 / year, based on education & experience
Care Managers are eligible for up to a $10,000 Sign-On Bonus!

Care Manager Position Responsibilities
- Works collaboratively with the multidisciplinary healthcare team to ensure the appropriate allocation of hospital resources while maintaining the quality of care.
- Participates in the utilization management process using standards of care to determine the most appropriate level of care, managing care across the continuum to ensure a safe discharge in a timely manner.
- Conducts the initial assessment of the patient's discharge plan based on the clinical course and proactively interacts with the healthcare team to establish the plan of care.
- Must have a current New York State Registered Nurse (RN) license.
- Graduate from an accredited nursing degree program required.
- Bachelor of Science degree in Nursing preferred.
- For RNs licensed in or after 2017, completion of a Bachelor’s of Nursing Degree will be required within 8 years of the date the license was obtained.
- Certification in a specialty area preferred.
- Three (3) - Five (5) years acute hospital experience.
- Case Management or Utilization Management experience preferred.
