Title: Case Manager
Area: Medical Respite
Provides short-term, intensive, site-based case management services to individuals experiencing homelessness who are recovering from acute illness or injury. Responsible for assessment, service planning, care coordination, and discharge planning in collaboration with Brown Health and internal teams to support safe recovery and successful transition from the program.
Hours:
Tuesday-Saturday 10am-6pm or Sunday – Thursday 10am – 6pm
Duties and Responsibilities:
· Advocates for the mission of the organization and upholds agency values
· Delivers services with safety, respect, and effectiveness
· Follows all policies and procedures of the organization
· Conducts comprehensive assessments and develops individualized service plans
· Provides ongoing case management, including goal setting, crisis intervention, and problem-solving
· Coordinates and communicates regularly with Brown Health providers and external partners to support care planning, treatment adherence, and discharge readiness
· Participate in interdisciplinary team meetings and case conferencing
· Supports clients in accessing medical care, behavioral health services, benefits, and community resources
· Supports day-to-day program operations as needed, including coordination of meals, laundry, room readiness, and responding to client needs in a shared living environment
· Leads discharge planning to ensure safe transitions to housing, shelter, or appropriate levels of care.
· Assist clients with housing applications, centralized waitlists, and landlord engagement
· Maintains accurate and timely documentation in HMIS and program records in compliance with requirements
· Maintain organized and confidential client files
· Engages clients using trauma-informed, harm reduction, and motivational interviewing approaches
· Responds to client crises, behavioral concerns, and incidents in accordance with program policies and de-escalation practices
· Manages a caseload of approximately 9–15 clients at a time, based on program census and acuity
· Promotes a safe, respectful, and structured environment within the Medical Respite Program
· Collaborates with internal staff and departments to support program operations and client needs
· Performs other duties as assigned
Requirements
Education and Experience:
- Bachelor’s degree in Human Services field OR
- Associates Degree in a Human Services field plus a minimum of 2 years experience in a Human Services field such as advocacy or case management OR
- A certification in Case Management plus a minimum of 2 years experience in a Human Services field such as advocacy or case management OR
- A minimum of 8 years experience in a Human Services field such as advocacy or case management.
Skills and Abilities:
- Ability to work in a fast paced environment.
- Must work well under pressure.
- Must be flexible and able to multi-task.
- Must be able to work effectively with populations that have experienced vulnerability, trauma, economic poverty, incarceration, substance use, developmental delays, compromised mental wellness, brain injuries, literacy and numeracy issues, and/or, other conditions or situations that have impacted housing stability.
- Must be solution-focused and organized in achieving objectives with all service users.
- Must be able to motivate and organize self to complete tasks with service users, often in non-therapeutic environments.
- Ability to maintain confidentiality in all aspects of the work environment.
- Excellent oral and written communication skills.
- Computer skills and data entry required.
- Requires reliable transportation outside of the public transit system.
