Support Services Director Qualifications & Core Responsibilities:
- Masters Degree in Medical Social Work
- Licensed Social Worker
- 2 years of Social Work experience in a hospice and palliative care setting.
- 2 years of experience as a supervisor with direct reports
- Clinical Social Work experience in dealing with geriatrics and/or adult population as well as a vast clinical knowledge base regarding the diseases, trajectory, and treatment of medical issues.
- Must have reliable, personal vehicle/transportation, a valid driver’s license, and the ability to safely and personally operate vehicles.
- As part of the clinical leadership team, the SSD must exhibit excellent analytical, clinical social work skills, leadership, and problem-solving skills, must have knowledge of the regulatory guidelines.
- Demonstrate excellent time management skills and detailed, task-oriented tendencies.
- Requires well-developed leadership skills, verbal communication, professionalism, and organizational skills and must be highly flexible,
- Will be required to enter and provide care inpatient residences as well as nursing facilities.
- Assist with the coordination/scheduling/monitoring and manage the daily workload of Support Staff (Chaplains, Social Workers, Bereavement and Volunteer Managers) providing direct patient care, with direct reporting to and supervision by the hospice Clinical Director.
- Assisting with the day-to-day leadership tasks that include, but is not limited to performing social evaluations, assessments to determine the appropriate service(s) for patients and provide ongoing EOL care
- Shift: Monday to Friday 8:30 am - 5:00 pm with after hours support required for team in the field
