About the Role:We are seeking a highly skilled RN Case Manager RNCM Hospice to join our team in the Health Care Services industry. As an RN Case Manager RNCM Hospice, you will be responsible for providing end-of-life care to patients in hospice care. You will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the highest quality of care. Your primary goal will be to manage patient care and ensure that patients receive the appropriate level of care based on their individual needs.
