About the Role:We are seeking a highly skilled and compassionate Registered Nurse to join our Home Health team in East Bay, CA. As a Registered Nurse in Home Health, you will be responsible for providing skilled nursing care to patients in their homes, ensuring their safety, comfort, and well-being. You will work closely with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans for each patient, and provide education and support to their families. Your ultimate goal will be to help patients achieve their highest level of independence and quality of life.Minimum Qualifications:Active and unrestricted Registered Nurse license in the state of CaliforniaMinimum of 1 year of recent experience in Home Health or related fieldCurrent CPR certificationExcellent communication and interpersonal skillsAbility to work independently and as part of a teamResponsibilities:Conduct initial and ongoing assessments of patients' health status and needsDevelop and implement individualized care plans based on patients' conditions and goalsAdminister medications, treatments, and procedures as ordered by physiciansMonitor and document patients' progress and response to careProvide education and support to patients and their familiesSkills:As a Registered Nurse in Home Health, you will utilize your clinical expertise, critical thinking, and problem-solving skills on a daily basis.
