About the PositionAs an Intake Coordinator with Crossroads Home Health and Hospice, you will work with our community partners and referral sources to help bring patients under our care. Responsibilities for this role include data entry, professional communication and coordination with patients and their families and working in a great team atmosphere at Crossroads. You will establish and maintain relationships while coordinating with hospitals and affiliates, care facilities, and other community partners as you promote excellent outcomes for our patients. We seek a candidate with excellent work ethic, organizational skills, attention to detail and an outgoing, charismatic personality.Location and Availability:This is a full-time position, Monday - FridayResponsibilities:Coordinate with community partners to process and accept referralsWork with the Intake department to ensure timely and accurate processing of referralsCollaborate with Community Liaisons and the Director of Business Development to develop relationships with community partnersCommunicate with patients and families about details of admissionSchedule visits for clinicians to go see patientsOther duties as assignedQualifications and Skills:Determined, collaborative, dedicated personality a must!Strong customer service skillsOne year experience in marketing/sales or in Home Health and/or Hospice preferredIntake experience preferredAbility to read a patient's chart is a plusExceptional interpersonal skills and an optimistic, friendly personalityExcellent communication skills, both verbal and writtenGreat organization skills with attention to detailTeam-oriented and self-motivatedComputer competent - must be able to handle technology with ease and learn new software systemsAbout UsCrossroads Home Health and Hospice is part of the Impact Healthcare family that currently provides Home Healthcare and end-of-life Hospice Care to 1,500 patients throughout CA, AZ, and UT.
