Crossroads Family Care is seeking a compassionate and dedicated **Licensed Professional Counselor (LPC)** to join our Behavioral Health team in East Texas. As a Federally Qualified Health Center (FQHC), Crossroads provides integrated medical, dental, and behavioral health services with a commitment to improving access to quality, patient-centered care.
The LPC will provide counseling and behavioral health services to individuals and families while working collaboratively with our interdisciplinary care team. This is an excellent opportunity for a counselor who values meaningful relationships, integrated care, and making a difference in the communities we serve.
What You'll Do
- Provide individual, family, and group counseling as appropriate
- Conduct behavioral health assessments and develop individualized treatment plans
- Provide evidence-based interventions for a variety of behavioral and emotional health needs
- Support patients experiencing depression, anxiety, stress, grief, adjustment concerns, and other behavioral health challenges
- Provide crisis assessment and intervention when appropriate
- Educate patients and families on behavioral health, coping strategies, and available resources
- Collaborate with medical providers and other members of the care team to support whole-person health
- Connect patients with community resources and additional levels of care when needed
- Maintain timely and accurate clinical documentation
- Participate in quality improvement and care coordination initiatives
Qualifications
- Master's degree in counseling or a related field from an accredited program
- Experience providing counseling in an outpatient, community health, or integrated healthcare setting preferred
- Strong clinical assessment, communication, and interpersonal skills
- Commitment to providing compassionate, culturally responsive, patient-centered care
- Ability to work effectively as part of an interdisciplinary healthcare team
