Brief Description:
Crossroads Community Services Board is currently seeking an experienced youth services case manager to coordinate care with children and adolescents in residential treatment and other community-based services. This unique position works closely with the Family Assessment and Planning Teams (FAPT) across all 7 counties that Crossroads serves. The incumbent will work to ensure that services recommended by FAPT are being provided as indicated and remain appropriate for the needs of referred youth. The position is also actively involved in assessing, discharge planning, and working with families to promote continuity of care between service providers and the youth's home. This role is ideal for a professional who is self-motivated, detail oriented, enjoys working in the field, and is passionate about helping children and teens experiencing mental health challenges. The successful candidate will have excellent written and oral communication skills and the ability to work cooperatively with diverse community partners.
