Patient Care Coordinator/ Case Manager

Crossroads Chiropractic is seeking a compassionate, organized, and energetic Case Manager to help guide patients through their care journey and ensure an exceptional patient experience from start to finish. We are looking for an incredible communicator with outstanding people skills, a passion for helping others, and the ability to inspire patients to take action toward better health.

Responsibilities:

• Welcome and support patients throughout their treatment plans
• Educate patients on recommended care plans and financial options
• Work closely with doctors and staff to ensure outstanding service
• Maintain accurate patient records while ensuring confidentiality
• Coordinate appointments, scheduling, and patient communications
• Help create a positive, encouraging office environment

Qualifications:

• Excellent communication and interpersonal skills
• Strong ability to connect with people and build trust
• Professional attitude with exceptional attention to detail
• Highly organized and able to manage multiple priorities
• Comfortable discussing care recommendations, finances, and patient goals
• Computer literacy and proficiency with office software
• Ability to work effectively in a team environment
• Prior experience in healthcare, patient coordination, customer service, enrollment, or B2C sales preferred
