Responsibilities and Duties

Duties Include But Are Not Limited To

Completing comprehensive assessments and collaborating with

individuals served to develop a care plan that is tailored to their

unique needs and goals.

Facilitating individual and group support that promotes growth,

safety, and empowerment.

Maintaining accurate and timely documentation of services provided.
Participating as a member of the interdisciplinary team to ensure

comprehensive coordination of care.

Crosspoint Human Services offers comprehensive on the job 40 hour

domestic violence training.
