Essential Job Functions
- Works with patients and family members utilizing evaluations and treatment techniques specific diagnostic categories and/or functional impairment. Provide individualized intervention and education utilizing task analysis based on identified learning needs of the patient to ensure optimal patient care.
- Responsible for completing documentation within established department standards to enhance effective patient care and to proper reimbursement.
- Records billing information of services provided.
- Interacts and practices in collaboration with a variety of professionals and health care consumers.
- Participates in departmental and multidisciplinary review of process, treatment or system issues for Quality Improvement initiatives.
- Assists with cleaning and maintenance of treatment area and department.
- Assists with maintaining inventory of stock and placing orders for the department.
- Complies with department competency list.
- Pursues relevant education opportunities to maintain or enhance professional knowledge and to meet departmental needs.
- Provides services in multiple settings (including skilled nursing, assisted living, patient homes, schools and outpatient) as needed or directed.
- Performs individualized activity assessments and follow-up visits with each swing bed patient to determine their interests and abilities and to develop an activity care plan.
- Other job duties and responsibilities as assigned to effectively meet the needs of the patients, the department, and the organization as a whole.

Requirements
Education
- Master’s Degree in Occupational Therapy unless grandfathered in at the Bachelor’s Degree Level: Required
Computer Skills
- Proficient in Microsoft Office
Certifications & Licenses
- Current State of Wisconsin License as an Occupational Therapist: Required
- Current State of Iowa License as an Occupational Therapist : Required
