- Enroll in and complete the CDHC Online Curriculum (tuition covered).
- Partner with the Chief Dental Officer to design and implement the CDHC program model.
- Identify and track patients with uncontrolled diabetes (A1C > 9) or HIV requiring dental engagement.
- Develop patient engagement and navigation processes to improve treatment adherence.
- Create culturally appropriate and multilingual educational materials on oral health.
- Track and analyze program outcomes using i2i population health dashboards.
- Present progress and outcomes quarterly to leadership using data-driven reporting.
- Coordinate with physicians, diabetic case managers, nutritionists, and pharmacy staff to ensure medical-dental integration (MDI).
