What You’ll Do
- Provide personalized primary care beginning with thorough history-taking and physical exams.
- Collaborate with the Interdisciplinary Team (IDT) on assessments, care plans, referrals & follow-up.
- Manage medications, reconcile interactions, and work closely with the Medical Director & Pharmacist.
- Monitor participants through hospitalizations and care transitions.
- Participate in daily IDT meetings and contribute to team-based planning.
- Complete assessments, documentation, labs/imaging reviews, and coding accurately and on time.
- Educate participants & caregivers on self-care, medications, and wellness.
- Provide coverage within the 24/7 on-call rotation.
- Support quality improvement, mentoring, clinical policy development, and best-practice initiatives.
- Step into other clinically appropriate roles to support team needs.
- MD, DO, APRN, or PA.
- Active New Jersey license (physician or nurse practitioner certification per NJ Board requirements).
- State CDS Certification & DEA Registration.
- Geriatrics board certification preferred.
- BLS required (within 90 days).
- 2 years of direct patient care preferred.
- Experience with frail or elderly populations preferred (training available).
- Strong history-taking, physical exam, diagnostic reasoning, and clinical documentation skills.
- Proficiency with EHRs, Microsoft applications, and interdisciplinary care models.
- Knowledge of PACE/CMS regulations, risk-adjustment documentation, and managed care principles.
- Reliable transportation, valid driver’s license & required auto insurance.
- Up-to-date immunizations & medical clearance.
- Ability to pass a comprehensive background check and drug screening.
- Commitment to people-first, person-centered care.
- Strong communication and collaboration with IDT members.
- Ability to manage complex medical situations and promote creative problem-solving.
- Understanding of diversity, inclusion, self-determination, and advocacy principles.
- High-level multitasking, organization, and adaptability.
- Adult Day Health Center & primary care clinic setting.
- Moderate noise level, controlled temperatures.
- Requires manual dexterity, ability to lift up to 30 lbs, and effective communication.
- Exposure to typical clinical risks (bodily fluids, infectious diseases, medical equipment, etc.).
