Job Summary
Our client is seeking two passionate Social Workers / Case Managers to play a key role in empowering adults to remain active, supported, and independent in their communities. This position provides hands-on case management, counseling, care coordination, and resource connection through in-person visits across Gaston, Cleveland, and Lincoln counties.
What You’ll Do
- Conduct assessments, care planning, and reassessments in partnership with the Interdisciplinary Team (IDT)
- Deliver individual, family, and group counseling and therapeutic activities
- Connect participants with community resources and advocate for service access
- Serve as a liaison between participants, families, and community organizations
- Maintain accurate documentation and update care plans as needs evolve
- Offer insight on social and cultural factors influencing care
- Support SMART goal setting and participate in 24-hour care coordination
- Contribute to social work programs and team collaboration
- Bachelor’s degree in Social Work required; LMSW, LISW, LCMSW, or LCSW preferred
- Three years’ experience in health and human services, ideally geriatric-focused
- One year of experience working with frail or elderly adults
- Strong counseling skills and knowledge of community resources
- Effective communication, documentation, and problem-solving abilities
- Valid North Carolina driver’s license and reliable personal vehicle
- Cleared medical and immunization requirements
