Job Details

• Position Description: Provides direct nursing care in accordance with established policies, procedures, and protocols of the healthcare organization.
• Responsibilities: Implements and monitors patient care plans. Monitors, records, and communicates patient condition as appropriate. Serves as a primary coordinator of all disciplines for well-coordinated patient care. Notes and carries out physician and nursing orders. Assesses and coordinates patients discharge planning needs with members of the healthcare team.
• Complexity of Work: Requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision. Must be able to work in a stressful environment.
• Unit Description: The skilled care unit offers a Swing Bed Program (Skilled Care Unit) that provides skilled nursing and rehabilitation therapies to help patients transition from an acute hospital stay back to home or a long-term care facility.
• Required Qualifications: Certifications, a valid Nursys License, Driver's License or State ID, adherence to Standard Precautions using personal protective equipment, and professional references.
• Schedule: Nights shift, 12-hour shifts, minimum of 36 hours per week.
