Long Term Care Nurse, LTC This position is located at a skilled nursing facility specializing in geriatric nursing, focused on providing supervised restorative care for older patients recovering from hospitalization or acute illness. The role involves delivering high-quality care to post-operative patients with various diagnoses such as pneumonia and congestive heart failure, ensuring optimal recovery and functional restoration.
Job Details
- Required Qualifications: WV Registered Nurse (RN) licensure, BLS Certification, minimum of 2 years of nursing experience in a clinical setting, experience working with post-operative hips, knees, shoulders, pneumonia, and CHF patients, and proficiency with Electronic Health Record systems, specifically EPIC.
- Schedule: 12-hour day shifts, with weekends and holidays rotating, minimum of 36 hours per week.
- Responsibilities include managing medication administration, physician orders, patient assessments, and collaboration with multidisciplinary teams to promote recovery and patient safety.
- Additional Information: The unit handles post-acute care with a patient ratio of generally 1:9-12, focusing on geriatric nursing and rehabilitation. Nurses will be responsible for admission procedures, physician order management, and ensuring quality patient care with an emphasis on safety and recovery.
