- Schedule: 8-hour days, starting as soon as possible, with a minimum of 40 hours per week.
- Additional Information: Traveler must have their own transportation due to occasional travel within a maximum radius of approximately 60 miles daily. The role involves patient visits, wound care, disease management, catheter and ostomy care, and G-tube management. The candidate must have completed the Oregon and Washington licensure. The facilitys charting is done via EPIC and Oasis systems remotely on a laptop and phone app.
- Float: Float rotation may be required, with the expectation for travelers to work across all facilities within the state.
- Certifications: BLS, Oregon State Licensure.
- Vaccinations: COVID vaccination (primary series) and one booster are required.
- Experience Requirements: 1 year of skilled home health experience.
- Submission Requirements: Three available interview times, confirmation of phone number, two references covering 1 year within the last 3 years, and indication if previously employed at the organization.
- Additional Notes: No on-call requirement. No weekend shifts. The role involves interfacing with interpretation services and requires compliance with facility policies.
