Job Summary:
Physical Therapist Home Health in an urban setting. This position involves providing therapy services across home health and skilled nursing facility environments. The role supports supervising physical therapist assistants and requires adapting to a dynamic schedule with diverse patient needs in a home-based setting.

Job Details:

• Schedule: Days shift, approximately 8 hours per day, 5 days a week, with guaranteed 40 hours weekly.
• Required Qualifications: Licensure to submit; BLS Certification; minimum of 1 year of experience; location requirements as per the state date.
• Preferred Qualifications: None specified.
• Additional Information: Therapy services are split roughly 50/50 between skilled nursing facilities and home health, with occasional coverage needed at other nearby facilities. Supervision of 2 physical therapist assistants is expected. Experience working with Net Health SNF and HCHB Home Healthcare software is preferred. No on-call or callback responsibilities are included. The assignment is scheduled to start on 09/28/2026 and end on 12/28/2026.
