Job Summary

Physical Therapist, Home Health. The position provides skilled home health services in a one-to-one patient setting, with visits grouped when possible to support efficient care delivery. The assignment includes eight-hour day shifts and may require weekend support.

Job Details

• Required Qualifications: At least 1 year of skilled home health experience; required OASIS experience; proficiency with Home Care Home Base (HCHB); active Oregon Registered Nurse (RN) license in hand before the start date; Basic Life Support (BLS) certification; reliable personal transportation and an active driver's license; timely communication and documentation; required completion of the Hospice/Home Health checklist.


• Schedule: Eight-hour day shifts, 40 hours per week; Monday orientation; weekend support may be required.


• Additional Information: Skilled home health setting with a one-to-one patient ratio; up to six patient visits per day; productivity of approximately 30 points per week; no call requirement; patient visits may be grouped to support efficient scheduling; documentation is completed using Home Care Home Base (HCHB), a remote home health client on a laptop, and the Rover phone application; business-casual attire is required, with scrubs permitted; requested time off must be submitted at the time of application.
