Job Details

• Position Description: Home Health Physical Therapist, serving the Northern Territory, including areas such as Mount Angel, Woodburn, Aurora, Molalla, Gervais, and Silverton. The position involves working days, with shifts typically lasting 8 hours.
• Required Qualifications: BLS certification, valid state license (including compact license accepted), at least 1 year of skilled home health experience, current COVID-19 vaccination series plus one booster, and reliable transportation.
• Schedule: Dayshift, 8 hours per day, minimum of 40 hours per week.
• Additional Information: Candidates are expected to have their own transportation, as travel between patient visits is required within a radius of approximately 60 miles per day. The role involves documentation via electronic systems including electronic patient charting using EPIC and Rover. Familiarity with OASIS and home health care regulations is preferred.
• Submission Requirements: Applicants must include responses to prior employment at Providence facilities and provide two references covering at least one year within the last three years.
