The role involves providing patient care within a designated territory, focusing on geriatric patients, with an emphasis on timely documentation and communication. The position requires a current license and relevant certification, with a flexible territory that may extend up to 60 miles from the hub. Job Details Qualifications: Valid license in hand, minimum of 1 year of relevant experience, proficiency in HCHB (Homecare Homebase), current BLS certification. Schedule: Days, 8-hour shifts, minimum of 40 hours per week. Special Requirements: Experience with OASIS for staff completing Start of Care in home health, active drivers license with personal transportation, and submission of RTO (Requested Time Off) at application. Additional Information: Timely documentation and communication are mandatory. All travelers are expected to be flexible with the territory and availability for an orientation on Monday. Travel pairs are accepted; each traveler must have own transportation. Modules are billable and completed during orientation. Submission: Must be submitted with an active drivers license, and the candidate must meet all relevant experience and certification requirements.
