Job Summary:
Physical Therapist (PT) specialized in Home Health (HH) services, located in a community setting. The position involves providing patient care for geriatric populations, with an emphasis on timely documentation, care coordination, and adherence to patient-specific treatment plans. The role supports home health services within a designated territory, requiring flexibility and professional accountability.
Job Details:
- Schedule: Mon - Fri; weekends optional; 8 hours per day; minimum of 40 hours per week
- Required Qualifications: Valid license in-hand; minimum of 0 years of experience; recent experience in home health; BLS certification; active drivers license with personal transportation; OASIS experience REQUIRED for staff completing SOC (Start of Care); proficiency in Homecare Homebase (HCHB) REQUIRED; timely communication and documentation; home health-specific certifications preferred
- Preferred Qualifications: Experience with geriatric patient care; familiarity with community hospital and LTAC settings; trauma level experience not required; previous travel experience beneficial
- Float: Float rotation may be required
- Additional Information:
- Start date: 10/05/2026
- End date: 01/04/2027
- Territory: Up to a 60-mile radius from the hub/office
- Travelers expected to be flexible with the territory
- All travelers are required to have an active drivers license, and personal transportation
- RTO (Requested Time Off) must be submitted at the time of application
- On-call rotation may be requireddiscuss during interview
- Modules are billable and completed during orientation
- No first-time travelers accepted
- All documentation and communication must be timely
- Monday orientation required
