Job Summary

Physical Therapist Outpatient, Logansport. The position evaluates assigned patients and develops, implements, and modifies individualized treatment plans in collaboration with physician direction. The role also provides patient education, supports functional recovery, and contributes to the delivery and development of discipline-specific therapy services.

Job Details

• Required Qualifications: Degree in Physical Therapy from an accredited school; at least 1 year of outpatient clinical experience; current Cardiopulmonary Resuscitation (CPR) certification.


• Schedule: Day shift, 8 hours per shift, Monday through Friday, with a minimum of 40 hours per week.


• Additional Information: Responsibilities include completing comprehensive patient evaluations, developing treatment goals, maintaining timely and accurate medical record documentation, providing care under valid physician orders, participating in student education when assigned, pursuing ongoing professional development, maintaining professional licensure in good standing, and adhering to applicable practice acts, regulatory requirements, safety standards, and professional codes of conduct.
