BCP Mobile Case Manager

Role Summary

The Mobile Case Manager provides intensive and individualized case management, including specialized trauma-informed and evidence-based interventions with youth. The individual in this position will create safe and welcoming environments for youth being referred to and enrolled in program services, with a focus on creating a positive relationship to engage and retain youth in supportive services. These individuals will be responsible for implementing evidence-based interventions with program fidelity and must be comfortable providing services in non-traditional settings with the goal of facilitating permanent connections with positive attachments, connections to school/employment opportunities, transitions to safe and stable housing, and improving the social and emotional well-being of youth.

Essential Duties and Responsibilities

  • Create a caring, safe, and healing environment that keeps the youth at the center of care throughout their experience with the Mobile Mansion (Recreational Vehicle).
  • Assist youth with completing safety planning, intake/enrollment/assessment, aftercare planning, discharge/follow-up forms and required agency and grant paperwork.
  • Coordinate with the referral staff/agency, determine the most appropriate location to complete the enrollment paperwork (community based and/or Crittenton Center), including the ACF data collection instruments and other screening and assessment tools.
  • Define strategies for communicating with the youth (face-to-face location in community, phone, email, text, and/or warm hand-offs at an office location with other referral agencies) to engage the youth and begin to build a positive relationship.
  • Meet with youth a minimum of 2–3 times per week at the location of their choice to ensure their needs are met and to offer the following: emergency shelter up to 21 days, counseling services, comprehensive support services, and access to gateway items/services. For youth who are uncertain or reluctant to accept services, intensive street outreach strategies will be developed to ensure youth are being met where they are at.
  • Develop positive relationships with community organizations and entities to promote Mobile Mansion services and connect youth with needed and available community resources.
  • Provide evidence-based interventions as appropriate and with fidelity.
  • Provide intensive case management services to assist in navigating other service systems to secure resources such as housing, transportation, food, and other practical items to aid families in their healing and recovery.
  • Follow up with clients and agencies as appropriate, per treatment needs.
  • Document client data accurately and completely, in accordance with Crittenton Center and ACF project documentation guidelines.
  • Work with grant staff to achieve and maintain client caseloads.
  • Participate in ACF and Crittenton Center trainings, community events, and other project activities as assigned.
  • Maintain client and employee confidentiality, abiding by all state and federal confidentiality expectations, including those established by HIPAA.
  • Contribute to the overall success of the grant by performing other duties and responsibilities as assigned.

Supervisory Relationships

This position does not have formal supervisory authority.

Required Education and Experience

  • Education and Experience:
    • This full-time position requires a bachelor’s degree in social work, psychology, or related field and/or someone with significant experience working and building rapport with vulnerable youth who have experienced complex trauma.
  • Personal Qualities:
    • Excellent communication skills, both verbal and written; excellent organizational and interpersonal skills; detail oriented.
  • Qualifications:
    • Must have a Valid Driver’s License and Auto Insurance
    • Must pass criminal background check, abuse registry check, fingerprint background check, and provide an up-to-date physical including TB test.

Position Type/Expected Hours of Work

This position is full-time, with general working hours of 8:00am-5:00pm. Standard days are Monday through Friday, with flexibility required to meet family and program needs; must be able and willing to work in non-traditional environments (streets, community locations). Some evening or weekend work may be necessary.
