What You’ll Do
As a Case Manager, you’ll play a key role in supporting clients through advocacy, case management, and access to community resources. Responsibilities include:
Coordinating and providing client-centered, safe, and effective care.
Managing cases, drafting service plans, and reviewing progress.
Advocating for clients and helping them make informed treatment decisions.
Conducting regular check-ins to track progress and ensure client satisfaction.
Maintaining accurate records and reports.
Participating in weekly team meetings, trainings, and agency fundraising events.
Networking with community agencies and supporting quarterly volunteer training.
What We’re Looking For
Bachelor’s degree in Social Work or related field or equivalent experience in human services.
Minimum of 1 year of post-internship experience in crisis counseling and case management (experience with crime victims preferred).
Bilingual (English-Spanish) required.
Strong interpersonal and organizational skills.
