What are the responsibilities and job description for the Director of Social Services position at Crest Nursing Home?
Primary and Essential Responsibilities for this position include the following:
Completes assists in admission, discharges and social service assessments, as part of the comprehensive resident assessment process. Assessments identify residents with current needs for social service interventions to improve or maintain functional abilities and those residents at risk of psycho-social deterioration. Completes RAI/MDS information as required. Works cooperatively with members of the interdisciplinary team to develop, implement and evaluate plans of care. Attends care conferences. Serves on all Committees. Audits clinical records of residents on a regular basis to monitor resident responses to care and treatment and to identify concerns about resident rights. Acts as role model for direct care staff in matters of protecting and promoting residents' rights. Suggests approaches and methods of interacting with residents and staff that maintain and enhance the person's dignity and individuality. Assists with preadmission and care planning for admissions. Arranges for social work or other mental health counseling services as needed to attain or maintain highest practicable mental and psycho-social well-being. Identifies ways to accommodate resident's choices, preferences and routines. Gives residents and prospective residents, family members and legal representatives information about the residents' rights in the facility. Advises on eligibility for Medicare and Medicaid benefits. Maintains ongoing contact with interested family members and legal representatives, with consent of resident, to inform them of changes in condition, discharge planning efforts, and to encourage family participation in developing plan of care. Evaluates residents for discharge potential. Provides discharge planning services that reflect the resident's and family's preferences for care, coordination of post discharge care and services, including transportation, and how the resident will access and pay for services. Actively participates in long term care survey process. Assist in marketing through outreach and public speaking engagements. Other duties as assigned.
Required Skills and Abilities include:
Degree in Social Work or human service field. Ability to communicate well both verbally and in writing. Past experience in a health care setting preferred. Must be free from convictions of abusing, neglecting, or mistreating individuals in a health care related setting.
