Job Overview
We are seeking a dedicated and skilled Registered Nurse (RN) – In-Home Infusion to join our healthcare team. The ideal candidate will have a strong clinical background and a passion for providing high-quality patient care in the home setting. This role focuses on antibiotic infusion therapy, including patient education and close monitoring to ensure safe and effective treatment. The RN will collaborate with physicians, pharmacists, and interdisciplinary team members to achieve optimal outcomes while promoting patient comfort and independence.
Key Responsibilities
- Administer in-home intravenous antibiotic infusions and monitor for therapeutic effectiveness and adverse reactions.
- Perform comprehensive patient assessments, including review of clinical status, vital signs, and lab results, documenting all findings accurately in the electronic health record (EHR).
- Initiate and maintain IV access, including venipuncture, peripheral IV insertion, and care of central venous access devices such as PICC or midlines.
- Educate patients and caregivers on infusion procedures, infection prevention, signs of complications, and proper line care.
- Adhere to strict aseptic technique and infection control protocols throughout all infusion-related procedures.
- Coordinate with prescribing providers and specialty pharmacies to adjust care plans, communicate changes in patient condition, and ensure continuity of antibiotic therapy.
- Manage medication administration and infusion pump setup, troubleshooting, and maintenance.
- Provide case management support to facilitate lab draws, follow-up appointments, and timely completion of therapy.
- Maintain HIPAA compliance and ensure confidentiality of all patient information.
Qualifications & Experience
- Current, unrestricted Registered Nurse (RN) license in the state of practice.
- IV infusion therapy experience with demonstrated competency in antibiotic administration and vascular access device care.
- Background in home health, infusion center, critical care, or medical-surgical nursing preferred.
- Proficiency with EMR/EHR systems (e.g., Epic, Cerner) and accurate documentation practices.
- Strong knowledge of infection control principles, antibiotic therapy monitoring, and potential adverse reactions.
- Excellent patient education and communication skills, particularly in explaining self-care and IV line maintenance.
Preferred Skills
- Experience caring for geriatric or immunocompromised patients.
- Familiarity with ICD-10 coding for accurate clinical documentation.
- Ability to work independently in a community or home-based setting, demonstrating critical thinking and sound clinical judgment.
