What are the responsibilities and job description for the Full Time Classroom Nurse position at CREST Collaborative?
CREST Collaborative is seeking an energetic LPN or RN to join our team who serves students in our medically fragile program.
The nurse is an integral part of the team which includes a classroom teacher, support staff and related service providers.
Responsibilities include:
Implement the plan of care for each individual student with chronic health conditions and disabilities.
Collaborate with health care providers and other school personnel to provide an optimal level of care to each student.
Provide direct nursing care as well as participate in students' activities of daily living.
Maintain comprehensive documentation of delivery of care and student heath records in Electronic Health Record (EHR), using SNAP computer program.
Maintain Medicaid documentation and submit monthly.
Work collaboratively with the school nursing team to support needs across student programs.
