POSITION SUMMARY (include key objectives, accomplishments or outcomes for the position):
Directs, manages and oversees CrescentCare’s Region 3 Houma office to ensure that client’s needs are addressed, and funder’s expectations are met. Provides supervision, training and support to the staff in Region 3. Oversees the day-to-day operational needs at the CrescentCare Exchange Support Service office/properties and works closely with the Chief Operating Officer and facilities staff to ensure the site's operational needs are being met. Ensures that staff are properly equipped to work with special populations including those living in rural areas with HIV, serious mental illness, histories of trauma and instability. Collaborates with local medical providers and CrescentCare Finance staff to maximize 340B revenue opportunities through medical case management. Works in collaboration with Behavioral Health leadership to ensure proper training and supervision of the behavioral health therapist. Maintains a close working relationship with direct supervisor, participates in active supervision and remains receptive to feedback on professional development to support high quality service provision.
RESPONSIBILITIES (The major functional areas Employees member is responsible for)
Provide strong leadership and effective management of the staff in CrescentCare’s Houma location. Ensure that a low barrier, person-centered approach to services is adopted and effectively implemented across all services provided in the office.
Responsible for managing the budget of the department.
Responsible for meeting grant and funder programmatic deliverables.
Manage the day-to-day of the physical buildings and operations.
Represent CrescentCare in the community. Create and maintain relationships with community partners and service providers.
Responsible for maximizing 340b revenue through medical case management.
ACCOUNTABILITIES (The “deliverables”, specific activities or key tasks Employees is expected to perform)
Ensure that staff implement program expectations and grant standards, including but not limited to intake and service provision, record keeping and documentation, work quality and productivity expectations, effective teamwork, and client-centered ethical practice.
Ensure that staff are meeting programmatic requirements around care, treatment, recovery plans, assessments, required paperwork, client engagement, and general procedures.
Monitor program targets including billing targets, no-show rates, scheduling availability, and program financial sustainability on a monthly basis to ensure deliverables are met.
Coordinate and monitor continuous quality plan based on reliable data with supervisor.
Assign and monitor client caseloads and staff workloads as needed.
Coordinate and implement interdisciplinary meetings as needed.
Provide direct clinical, administrative, and supportive supervision to all assigned staff both at scheduled routine intervals and throughout the workday, including routine feedback on performance and behavior.
Formally evaluate performance annually; establish and implement parameters for quantitative evaluation.
Ensure staff has the needed and required training to effectively carry out responsibilities, including training working with special populations as needed (e.g. SMI, HIV, CCM, etc.) and providing needed interventions (MI, CBT, TIC, MHR. CPST, PSR, CI, etc.)
Collaborates with Behavioral Health leadership to ensure proper training and supervision of the clinical aspects of the work of the behavioral health therapist, provides administrative oversight of the therapist.
Ensure that the program is appropriately staffed, conduct applicant searches and interviews as needed.
Provide timely coaching and/or disciplinary action when staff are not meeting performance or behavioral expectations and standards.
Review departmental budget and grant budgets at least monthly.
Participates in yearly budget process and tracking of assets.
Collaborates with CrescentCare Finance team in developing the service budgets and contracts for ESS Ryan White and HOPWA programs.
Ensures reports and requirements for funders are completed and submitted timely. (HOPWA, Ryan White Part B, Medicaid, etc.)
Provide daily oversight of the operations aspects of 104 and 106 Graham Douglass in terms of ensuring building upkeep, maintenance and communicating with CrescentCare Information Services and Operations staff when support is needed.
Collaborates with local medical providers and CrescentCare Finance staff to maximize 340b revenue opportunities through medical case management.
Organize program meetings, team meetings, and staff in-services.
Actively participate in CrescentCare leadership meetings and activities. Coordinate and collaborate with other leaders.
Create and maintain program SOPs as needed.
Coordinate and implement monthly group meetings with a clinical focus (case review, consultation, etc.) with direct reports.
Provide direct services to clients on caseloads when there is a case manager vacancy or when necessary.
Ensure that requirements from accreditation bodies are met (CARF).
Attends a minimum of 20 hours of continuing education annually.
POSITION REQUIREMENTS
Education
Master’s degree in social work (MSW) or equivalent area of study at graduate level is required.
Work Experience
Ten years of direct social work practice experience is required.
Five years of experience providing direct supervision of social work staff is required.
OTHER, (licensure, certifications)
Minimum age of 21 is required.
Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) is required.
Reliable transportation and a valid driver's license is required.
Must have no automobile license denials, revocations, or more than one suspension in the past three years and valid car insurance.
No more than one at-fault automobile accident in the past three years
No more than two moving violations in the past five years.
PHYSICAL DEMANDS OF POSITION
Extended periods of sitting is required.
Ability to work in office in a primarily sedentary position is required.
Travel to other CrescentCare locations is required.
Ability to lift up to 25 pounds is required..
