Position Summary
The Home Health Registered Nurse (RN) Case Manager is responsible for providing direct skilled nursing care in the patient’s home, developing individualized care plans, completing Medicare-compliant OASIS assessments, coordinating interdisciplinary services, and managing clinical documentation. This role combines autonomous field nursing with clinical case management responsibilities.
Essential ResponsibilitiesPatient Care & Skilled Nursing
- Provide skilled nursing visits to patients in their homes.
- Perform assessments, wound care, injections, IV therapy, catheter care, diabetic management, chronic condition monitoring, and patient/family teaching.
- Develop and implement a personalized Plan of Care based on patient needs and physician orders.
- Monitor clinical progress and adjust care plans accordingly.
OASIS & Documentation
- Complete timely and accurate OASIS assessments: SOC, ROC, Recert, Discharge, and Transfers.
- Document clinical encounters in the EMR (e.g., Axxess)
- Ensure Medicare compliance, coding accuracy, and timely documentation submission.
Case Management
- Coordinate services with PT, OT, ST, MSW, and Home Health Aide.
- Communicate regularly with physicians, hospitals, SNFs, and family caregivers.
- Participate in case conferences, quality reviews, and clinical meetings.
- Ensure patients receive appropriate visit frequency and skilled interventions.
- Track progress on high-risk patients and implement interventions to reduce hospital readmissions.
Qualifications
✔ Active RN license in the State of Illinois (or relevant state)
✔ Minimum 1 year of clinical RN experience (hospital, SNF, or home health preferred)
✔ Prior home health or community nursing experience highly preferred
✔ Experience with OASIS documentation (SOC, ROC, Recert, Discharge) preferred
✔ Ability to work autonomously in the field and manage a clinical caseload
✔ Strong communication and teaching skills
✔ Valid Driver’s License and reliable transportation
Expected Productivity (Adjust as Needed)
- Manages 15-20 active caseload patients depending on acuity
- Performs 18–22 visits/week including SOC, follow-ups, and recerts
- Completes documentation within required compliance timelines

Pay: $40.00 - $48.00 per hour
