POSITION SUMMARY: The Speech-Language Pathologist contracted or employed through the agency is responsible for the implementation of standards of care for speech therapy services and for adherence to all conditions outlined in the Job Description or Independent Contractor Agreement.
ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:
Conducts appropriate evaluations, assesses home environment, and identifies equipment needs relative to speech/language function as needed.
Assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, family and caregiver.
Provides services that are ordered by the physician as indicated in the plan of care.
Provides patient, family, and caregiver, counseling.
Provides patient, family, and caregiver education.
Prepares clinical notes.
Communicates with the physician who is responsible for the home health plan of care and other health care practitioners, as appropriate, related to the current home health plan of care.
Identifies functional speech-language deficits and establishes a plan of care to improve patient's function.
Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.
Recommends communication devices/aids as indicated.
Participates in required case conferences, in-services and other meetings to ensure coordinated and comprehensive care.
Maintains clinical competency in speech language pathology practice and theory.
Identifies patient, family, and caregiver needs for other home health services and refers as appropriate.
May supervise Home Health Aide in procedures directly related to the speech-language therapy plan of care.
Participates in agency's quality assessment and performance improvement program and agency-sponsored in-service trainings.
Other appropriate services and special projects as assigned.

QUALIFICATIONS & REQUIREMENTS FOR THE POSITION:
Current SLP licensure in state of practice. If state licensure not required, must have completed or be in the process of completing supervised clinical practicum of not less than nine (9) months full-time.
At least one (1) year of clinical experience working as a Speech-Language Pathologist in health care, home health experience preferred.
Current CPR certification required.
Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
PREFERRED KNOWLEDGE AND SKILLS:
Excellent time management and problem-solving skills.
Must have outstanding verbal and written communication skills.
SUPERVISORY RESPONSIBILITIES:
May supervise HHA as needed.
