Duties
- Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations to determine individual care needs.
- Administer medications via injections, IV infusions, and other routes following aseptic techniques and medical documentation standards.
- Provide specialized nursing interventions such as wound suturing, ostomy care, catheterization, ventilator management, and tube feeding.
- Manage complex cases involving acute pain management, infectious disease care, hospice and palliative medicine, and behavioral health support.
- Collaborate with interdisciplinary teams to develop personalized care plans aligned with medical records systems like Epic or Cerner; ensure compliance with HIPAA regulations.
- Educate patients and families on health coaching, medication administration, disease management including Alzheimer’s or dementia care, and post-acute discharge planning.
- Perform laboratory specimen collection/processing and maintain accurate medical records within electronic health record (EHR) systems for seamless documentation.
Requirements
- Valid LPN or RN license in good standing; experience working with individuals with disabilities or in geriatrics strongly preferred.
- Demonstrated proficiency with EMR/EHR systems such as Epic or Cerner; familiarity with ICD coding (ICD-9/ICD-10) is advantageous.
- Critical care experience including ICU or trauma center exposure highly desirable; background in hospital medicine or emergency medicine is a plus.
- Strong knowledge of medical terminology, anatomy, physiology, aseptic techniques, and infection control protocols.
- Experience working with diverse populations including pediatric (pediatrics), neonatal (neonatal ventilator), adult (internal medicine), and long-term care settings such as nursing homes or assisted living facilities.
- Ability to perform physical examinations, venipuncture (phlebotomy), IV insertion/infusion therapy, catheter care, and specimen collection accurately.
- Excellent communication skills for triage assessments, patient education, discharge planning, and case management responsibilities.
