Community Outreach Specialist

Credible Health Solutions is seeking a motivated and outgoing Community Outreach Specialist to support the growth of our Mental Health Targeted Case Management (MHTCM) services in the Houston area.

This role focuses on building strong relationships within the community, increasing awareness of Medicaid-covered behavioral health services, and generating referrals by connecting eligible children, adolescents, and families to Skills Training and Targeted Case Management services through outreach and networking efforts.

The ideal candidate is self-driven, enjoys working in the field, and is comfortable engaging with individuals, families, schools, and community partners. This position plays a key role in expanding CHS’s community presence by developing referral sources, conducting outreach activities, and supporting the intake process by ensuring referrals are properly submitted and followed up on.

This position offers immediate eligibility for performance-based bonuses tied to client enrollments, as well as the opportunity for increased pay after a 90-day performance review.

This role does not provide direct client services and is strictly focused on outreach, referral generation, and community engagement.

• Conduct community outreach to increase awareness of Credible Health Solutions services, including Mental Health Targeted Case Management (MHTCM)
• Build and maintain professional relationships with community partners, including schools, clinics, hospitals, and local organizations
• Identify, develop, and maintain new referral sources throughout the Houston area
• Attend and actively participate in community events, health fairs, school meetings, and networking opportunities
• Educate individuals, families, and community partners about Medicaid-covered behavioral health services, including Skills Training and Targeted Case Management
• Support the client enrollment process by connecting interested individuals with the intake team and ensuring referrals are properly submitted
• Maintain consistent follow-up with referral sources and community contacts to promote ongoing engagement and referrals
• Track and document outreach activities, contacts, and referral efforts in accordance with company expectations
• Collaborate with internal staff to ensure a smooth and timely onboarding process for new clients
• Travel throughout the Houston area to conduct in-person outreach and relationship-building activities
• Meet and maintain weekly and monthly outreach goals related to referrals and client engagement

Pay: $14.00 - $16.00 per hour
