Creative Community Living Services (CCLS) is currently in need of a Full-Time, 2nd Shift Caregiver for a 20 bed Assisted Living Facility located in Bayside, WI.

Hours Available:
- WK 1: Sun/Tue-Fri (2pm-10pm)
- WK 2: Mon-Thurs and Sat (2pm-10pm)
- Opportunities to pick up shifts
Responsibilities:
- Assist residents with personal cares and leisure time activities
- Assist with residents taking prescribed medications
- Follow all DHS and CCLS policies, rules and regulations
- Deliver care as prescribed in each care plan
- Record all activity in the medical record
- Adhere to proper documentation
Qualifications:
- High School diploma or GED.
- Six months of experience in an assisted living facility or experience with direct care in any environment.
- Are able to learn and use care plan computer software
- Acceptable criminal background information disclosure
- Acceptable pre-placement drug screen
