Job Summary: This position provides case management to children with serious emotional disorders, and their families to ensure access to culturally appropriate resources required for a stable, safe and healthy community life.
Duties and Responsibilities:
- Caseload 20-25 per child worker
- Provide consumer contact bi-weekly or monthly as designated by MBH's environment matrix updated every six months.
- Provide assistance to consumers to identify, access, and learn to use community resources.
- Provide initial intake and referral.
- conduct strength based assessments, within thirty days of registration, annually thereafter.
- Conduct service planning with thirty days of registration, every three months thereafter.
- Assist in developing informal network supports.
- Provide crisis interventions.
- Attend trainings as required.
- Interact with appropriate funding sources.
- Maintain accurate record keeping and documentation of all contacts.
- Assist in identifying gaps in service for purpose of program development.
- Perform all other duties as requested by the supervisor.
Skills, Knowledge, Training, Education, And Experience Required:
Credentials per Regulations: Must have one of the following:
- A Bachelor's Degree with major course work in sociology social work, psychology, gerontology, criminal justice, theology, nursing, counseling, or education.
- Be a Registered Nurse
- A High School Diploma and twelve semester credit hours in fields of study previously identified and two year's experience in public or private human services with one year in direct client contact.
- A High School Diploma and five years of mental health direct care experience in public or private human services with employment as an ICM staff person prior to April 1, 1989.
- Must have a Valid Driver's License.
