Overview
Creative Foundations LLC, a new and vibrant early intervention agency is seeking well-rounded and compassionate Service Coordinators to join our team. The Service Coordinator will work closely with families, therapists, and other stakeholders to facilitate effective rewarding early intervention experience for families.
Responsibilities
- Conduct initial visits with families interested in the voluntary NYS Early Intervention Program
- Assist families in the Early Intervention process through connecting families with vital resources
- Participate in IFSP process and meetings for families of eligible children
- Assist and guide families through the CPSE transition process
Experience
- Bachelor's Degree in Human Services or Health Sciences Field
- Associate's Degree in Human Services or Health Sciences field and one year service coordination experience
- Two Years of Service Coordination and/or one year service coordination experience and one year infant/toddler experience.
- Strong organizational skills with the ability to manage multiple cases simultaneously while maintaining attention to detail.
- Excellent communication skills, both verbal and written, to effectively interact with clients and team members.
- A commitment to ongoing professional development in the mental health and/or early childhood field is highly valued.
- A positive attitude
- Monolingual and Bilingual languages are welcomed.
Job Types: Full-time, Contract
Pay: $25.00 - $40.00 per hour
