POSITION OVERVIEW
Caregivers are responsible for assisting clients on a One-on-One basis, and whom are in need human services. Caregivers support goal-setting, personal growth, and behavior modification to their clients. Caregivers primarily assist clients by strategizing the needs of the client to the human services available, and executing a support plan that will help the client achieve self-sufficiency. Caregivers are expected to provide clear and coherent daily routines that correspond to the individuals Individual Support Plan.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
- Responsible for providing personal support service, which may include assistance in maintaining physical space, equipment to insure a high quality and safe living environment, and 24-hour emergency assistance.
- Monitors the health and safety of all individual's.
- Implement the individuals objectives and behavioral plans specified in the Individual Service Plan.
- Documents all support services according to client ISP.
- Submit consumer notes detailing which ISP goals were focused on during a shift.
- Assist their client with activities of daily living. Assist in keeping accurate data on each individuals progress.
- Supervise the individual on all their tasks, training and community activities.
- Assist individuals on all household chores, yard maintenance and keeping their home cleanly.
- Performs daily cleaning and vacuuming of the home.
- Attends required agency staff meetings and in-service trainings.
- Appropriately follow company protocol to administer medications to individuals.
- Properly document each self-dispensing administration in the Medication Administration Record.
- Responsive to emergency needs; reports and assist in the time of emergency.
- Has superior attendance.
- Communicates all attendance concern with supervisor or Human Resources.
- Always ensure client supervision during shift.
- Maintain appropriate and professional relationships with peers, consultants, individuals, and the community.
- Reports all possible cases of abuse, neglect or endangerment to appropriate supervisor; complies with all mandated reporter legal requirements.
- Assumes continuing responsibility for maintaining an attractive, interesting area in which to work with clients; shares in performance of routine housekeeping, monitoring and clerical tasks.
- Maintains a professional code of ethics; and Performs related work as required.
QUALIFICATIONS
Education and/or Experience
- High School graduate or equivalent (required).
- Minimum 6 months formal experience providing direct supervision for individuals with development disabilities (Preferred).
Required Licenses/Certifications
- Valid CA Driver License (Required).
