Position Overview:
In this role, you will provide case and resource management for adults who have Intellectual and/or Developmental Disabilities. Identify the support needs of clients, and plan for and authorize payment for appropriate services, schedule and conduct In-Person visit with clients. Complete and revise client assessments and service plans within the established timeline as required for caseload review and monitor client plans, reports, and client progress. This position is mobile and may require commuting to Blythe, 29 Palms, Barstow, Ontario, Temecula, Indio, and other locations required.
Essential Duties and Responsibilities:
- Coordinate client-focus treatment and service that are personalized, timely, and effective.
- Handle each case with understanding and a goal of helping clients achieve wellness and autonomy.
- Drive change by identifying areas where performance improvement is needed.
- Achieve organizational goals while upholding standards and best practices.
- Strong verbal and written communication skills to explain to clients and professionals the case and care plan and maintain excellent case records.
- Critical thinking and problem solving to determine the best care plan for each client after assessing clients, analyzing notes from health care and social workers.
- Flexibility to change care plans if they are not getting the best results.
- Organization to manage several different cases simultaneously.
- Computer literacy to maintain and manage case records.
- Must be willing to commute to client locations.
- Ensures compliance with established internal control procedures by examining records, reports, operating practices, and documentation.
Education and/or Experience:
- Minimum Two (2) years of Case Management Experience (Required)
- Valid CA Driver's License (Required)
