What You'll Do
- Manage medication therapy for oncology and hematology patients
- Partner with oncologists, infusion nurses, and care teams to optimize treatment plans
- Educate patients on chemotherapy, immunotherapy, and supportive care medications
- Monitor treatment response and recommend therapy adjustments when needed
- Review medications for safety, accuracy, and potential drug interactions
- Provide clinical guidance and therapeutic recommendations to providers
- Active Pharmacist license
- Infusion experience required
- 2 years of clinical pharmacy experience, preferably in oncology or hematology
- BCOP certification preferred
- PharmD preferred
