Role Overview
The MDS Coordinator is responsible for managing and overseeing the Minimum Data Set (MDS) process in a long-term care facility, ensuring accurate documentation, regulatory compliance, and proper reimbursement (Medicare/Managed Care). This role reports to the Director of Nursing and works closely with clinical, administrative, and admissions teams.
Key Responsibilities Lead and manage the full MDS and RAI process, including assessments, care planning, and submissions Ensure accurate, timely documentation to support Medicare and regulatory compliance Coordinate with interdisciplinary teams to gather clinical data and maintain care plans Monitor Medicare eligibility, certifications, and recertifications Oversee completion, accuracy, and submission of all MDS assessments Track and manage RUG classifications and reimbursement-related data Conduct audits and ensure documentation meets state, federal, and Medicare standards Participate in admissions and initiate MDS/care plans within required timeframes Train staff on documentation and MDS processes Communicate issues and updates to leadership Qualifications Licensed RN or LPN Experience in long-term care with strong knowledge of MDS, RAI, OBRA, Medicare, and case mix Strong organizational, communication, and leadership skills Ability to meet deadlines and manage compliance requirements Computer proficiency Physical & Work Requirements Ability to lift up to 50 lbs, stand/walk frequently, and perform physical tasks High exposure risk to communicable diseases Full-time (40 hours/week), hourly position Core Competencies Attention to detail and regulatory accuracy Leadership and team coordination Clinical knowledge and documentation expertise Commitment to resident care and compliance standards,
