What You'll Do
- Develop and maintain relationships with physicians, medical practices, hospitals, rehabilitation facilities, senior living communities, elder law attorneys, and other healthcare professionals
- Educate community partners on the organization's RN Care Management services and proactive approach to aging
- Meet with families and healthcare professionals to understand care needs and explain available solutions
- Identify seniors and families who may benefit from additional support, monitoring, care coordination, or other services
- Conduct community outreach, presentations, networking activities, and marketing initiatives
- Attend community events, including occasional evening and weekend events as needed
- Create and publish blogs and other marketing content to support community education and outreach
- Serve as a knowledgeable healthcare resource and trusted representative of the organization
- Collaborate with clinical teams to ensure clear communication and appropriate care recommendations
- Maintain consistent follow-up with referral sources and community partners
- Track relationship development activities and referral opportunities
- Build long-term partnerships throughout the Sarasota healthcare and senior care community
- Registered Nurse (RN) strongly preferred
- LPN, healthcare professional, or experienced senior care professional backgrounds will also be considered
- Strong understanding of senior care, chronic conditions, and healthcare navigation
- Excellent verbal and written communication skills
- Strong interpersonal and relationship-building abilities
- Professional, polished, and confident when presenting to physicians, executives, families, and community groups
- Warm and empathetic approach with the ability to establish trust quickly
- Strong organizational skills with the ability to manage multiple relationships and priorities
- Self-motivated with the ability to independently manage field-based responsibilities
- Comfortable networking, attending community events, and developing professional partnerships
