Description

The Licensed Behavioral Social Worker (LBSW) provides community-based behavioral health services, case management, psychosocial rehabilitation, crisis intervention, and care coordination for children, adolescents, and families experiencing emotional, behavioral, mental health, and substance use challenges. This position delivers goal-directed, solution-focused interventions designed to promote recovery, resilience, emotional well-being, and successful functioning in home, school, and community environments. Services are provided in accordance with individualized treatment plans, behavioral health goals, and applicable regulatory requirements.

The LBSW conducts assessments, develops and implements service plans, facilitates skill-building interventions, provides crisis response and de-escalation services, and coordinates care with families, schools, healthcare providers, and community partners. The position advocates for client access to needed resources and support while assisting individuals and families in overcoming barriers that impact treatment progress and community stability. Services are delivered in homes, schools, agency facilities, and other community locations where clients live, learn, and socialize.

The LBSW collaborates closely with clinical staff, medical providers, educators, and other treatment team members to ensure coordinated, strength-based, and recovery-oriented service delivery. The position is responsible for maintaining timely and accurate documentation, meeting productivity expectations, adhering to agency policies and professional standards, and supporting the mission and values of Crawford County Mental Health Center.

Requirements

Primary Job Duties and Responsibilities:

  • Provide regularly scheduled social work encounters and rehabilitation services in accordance with treatment plans, service plans, and Individualized Education Programs (IEPs), when applicable.
  • Document all services in the school system on the same day as the service and the agency Electronic Health Record (EHR) according to documentation standards
  • Assist clients in implementing tasks and interventions related to treatment plan goals.
  • Facilitate psychosocial rehabilitation individual or groups, and social skills development groups utilizing evidenced-based programs.
  • Provide individual social, emotional, behavioral, and problem-solving skill development services.
  • Monitor client progress toward goals and modify interventions as appropriate.
  • Coordinate behavioral health goals and interventions with parents, guardians, school personnel, healthcare providers, and other members of the treatment team.
  • Collaborate with school staff, therapists, psychiatric providers, medical personnel, state hospitals, and community agencies to support continuity of care and positive client outcomes.
  • Assist clients and families in accessing community resources, supports, and benefits, provide information and education, or transportation when appropriate and authorized.
  • Advocate for clients to ensure appropriate services, supports, and benefits are obtained and maintained.
  • Respond to psychiatric, behavioral health, and emotional crises through crisis intervention, de-escalation, stabilization, and referral services under direction of a CMHC QMHP.
  • Coordinate referrals, transitions of care, discharge planning, and follow-up services with internal and external providers.
  • Deliver services in community settings including homes, schools, and agency facilities.
  • Maintain compliance with agency policies, Medicaid regulations, HIPAA requirements, accreditation standards, and applicable federal and state regulations.
  • Participate in staff meetings, supervision, training, quality improvement activities, and professional development opportunities.
  • When school is not in session the employee will report to the Youth Crisis Stabilization Unit or Challenger program to provide direct care.
  • On School days, assist the Challenger program with groups after school.
  • Perform other duties as assigned.

Credential And Experience Requirements

  • Bachelor's degree in Social Work from an accredited college or university required.
  • Current Kansas Licensed Bachelor Social Worker (LBSW) license in good standing preferred.
  • Ability to secure Kansas Bachelor's Level Social Worker licensure within 60 days of employment.
  • Experience working with children, adolescents, and families preferred.
  • Experience in community mental health, crisis services, school-based services, or behavioral health settings preferred.
  • Valid Kansas driver's license and acceptable driving record required.
  • Required agency certifications and annual training must be maintained.

Required Skills/Abilities

  • Knowledge of community-based behavioral health services and psychosocial rehabilitation.
  • Knowledge of crisis intervention, stabilization, recovery-oriented care, and trauma-informed practices.
  • Knowledge of treatment planning, case management principles, and community resources.
  • Ability to build effective relationships with clients, families, schools, providers, and community partners.
  • Strong communication, organizational, time management, and documentation skills.
  • Ability to work independently while maintaining collaboration with interdisciplinary teams.
  • Ability to maintain confidentiality and comply with HIPAA requirements.
  • Ability to effectively manage challenging behaviors and crisis situations.

Incumbent Must

  • Be able to communicate effectively in spoken and written English language.
  • Possess a valid driver's license.
  • Operate usual office equipment.
  • Ability to operate effectively within a wide variety of human service situations.
  • Be able to lift and carry up to 25 pounds occasionally.
  • Have the ability to move freely in the community and in a wide variety of terrain and circumstances. Special accommodation that can successfully assist an incumbent will be provided.

  • Ability to communicate in languages other than English.

  • Knowledge regarding self-help programs of recovery.
