Job Title: RN / LPN for Home Health Supervisory Visits (PRN / Per Visit)
Job Summary: We are currently seeking a compassionate and reliable Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team on a PRN/Per Visit basis. You will be responsible for conducting in-home supervisory visits for clients enrolled in the Medicaid Waiver programs. As our agency continues to grow, there will be potential opportunities for additional work and expanded nursing roles with us in the future.
Important Note for Applicants: Please note that as part of our initial Medicaid Waiver provider enrollment process with the state, we are required to employ a nurse. Therefore, we will need a copy of your current Alabama Nursing License and a record of your earned CEUs to complete our provider application.
Key Responsibilities:
- Conduct in-home supervisory visits for all assigned clients every 60 days.
- Assess client needs, monitor care, and ensure compliance with the E&D and ACT Waiver program standards.
Requirements:
- Current, active, and unencumbered Alabama RN or LPN License.
- Ability to pass a comprehensive national background check, including the National Sex Offender Registry, Nurse Aide Registry, and OIG Exclusions Database.
- Reliable transportation and willingness to travel to clients' homes.
Job Type: Part-time, PRN / Per Diem
Pay: $23.00 - $25.00 per completed supervisory visit
Pay: $23.00 - $25.00 per hour
Expected hours: 1.0 – 5.0 per week
