The Opportunity:
We are looking to add a Palliative Care Nurse Practitioner (NP) to our team to provide in-person care for patients in Boulder, Longmont, Broomfield and the surrounding communities.
The Nurse Practitioner must:
- Be a graduate of an accredited school of nursing
- Be currently licensed as a Nurse Practitioner in Colorado
- Have Prescriptive Authority (RXN) by the Colorado Board of Nursing
- Have a minimum two years prior Nurse Practitioner experience, preferably in palliative or hospice care, managing chronic pain and symptoms related to serious illnesses
- Possess strong clinical assessment and diagnostic skills and a demonstrated ability to manage complex patient cases
Responsibilities include:
- Performing in-person initial and ongoing assessments on each assigned palliative care patient and developing the plan of care in collaboration with other healthcare providers.
- Providing education and counseling to patients and families about illness progression and available treatment options.
- Participating in interdisciplinary team (IDT) meetings to review and update patient care plans.
- Accessibility 24 hours/day for patient care interventions.
Multiple days a week of availability is preferred.

Pay rate: $60-$70/hour (dependent upon experience)
