What are the responsibilities and job description for the Licensed Practical Nurse-Floater position at CP Unlimited?
WHAT WE ARE LOOKING FOR:
Looking for a courteous and efficient Licensed Practical Nurse who is able to provide a positive experience for a special needs patient population and their caregivers at all of our FQHC locations. Candidate must be licensed and hold a valid NYS Registration Certificate. BLS Certification is also required.
Responsibilities will include:
- Prepares daily assignments for nursing staff
- In collaboration with the Director of Nursing, establishes and implements systems for day-to-day medical and nursing operations
- Participates as a member of the Patient Centered Medical Home Care team. Participates in daily huddles and practice team meetings, and ensures staff compliance with all aspects of the PCMH initiatives
