The Primary Care Health Home (PCHH) Ambulatory Care Coordinator I plays a vital role in supporting and advocating for patients and their families as part of an interdisciplinary health care team, helping to overcome barriers to the progression of care. Working under indirect supervision in both clinical and community-based settings, the ACC is responsible for a range of duties, including communicating with patients regarding open care gaps, scheduling appointments, coordinating resources, and assessing social determinants of health. The role requires a team-oriented individual with strong communication, problem-solving, and organizational skills.
